• his/her estate; • a person age 18 or over; • a benevolent, educational, eleemosynary, charitable or religious organization or institution. Serving members may obtain a beneficiary form from their Base Personnel Office. Pensioners should contact DCFPS, to obtain a beneficiary form. A pensioner does not have to complete a new beneficiary form after being released from the CF. The beneficiary form which was completed during their service will remain valid until the pensioner chooses to change the beneficiary. A CF member/pensioner may change their SDB beneficiary at any time. The most recent beneficiary form on the CF member/pensioner’s pension file at the time of death, will be the form used to determine the recipient of the SDB. Note that if there is no beneficiary form on file and the member was in the regular force and married prior to 20 December, 1975 the death benefit will be paid directly to the survivor.
