You do. You pay for all shipments: you pay for the blank forms shipped to you, you pay when you ship the forms back to us, and you pay for the printed reports and your answer forms shipped finally to you. Your invoice will include S&H charges for our two shipments to your school: first when we send your blank forms, then when we send your reports.
