Who needs to complete the Immunization Form/Report of Medical History?
All incoming students enrolled to the University for the fall and/or spring semester and will be attending classes on the ECU campus. (Students who are attending classes off-campus, auditing classes, taking Internet classes, or satellite classes DO NOT have to complete these forms. Exemption is automatically given at time of course registration.
