Who needs a Medical Report for an HGV/LGV/PSV Licence?
The Medical Examination Report form D4 has to be completed by: • Anyone who is making their first application for a license to drive a vehicle over 3.5 tonnes or a public service vehicle with more than 8 seats; • Anyone renewing their LGV/PSV entitlement aged 45 or over; • Anyone aged 45 or over will have to renew their license every 5 years, then every year after the age of 65. Some medical conditions will result in a licence being granted for 1, 2 or 3 years. The DVLA should give notice prior to the renewal date. Medicals usually take 60 minutes and involve a long questionnaire followed by an eye test, blood pressure measurement, urine test and a full physical examination of heart, lungs, circulation, nervous system, musculoskeletal system and abdomen. The D4 form will be completed after the medical examination ready to return to the DVLA.
