The medical Examination Report Form D4 has to be completed by: • Anyone who is making their first application for a licence to drive a vehicle over 3.5 tonnes or a public service vehicle with more than 8 seats. • Anyone renewing their LGV/PCV entitlement aged 45 or over; • Anyone aged 45 or over will have to renew their licence every 5 years, then every year after the age of 65. • Some medical conditions will result in a licence being granted for 1, 2 or 3 years. The DVLA should give notice prior to the renewal date.
