The parent, legal guardian, or legal custodian of any child who has reached the age of 6 and has not yet turned 16 by September 15 of the current school year, and who is not enrolled in either a public school or an accredited non-public school must file a CPI Form A annually, in duplicate to the school district of residence. If you change your district of residence, you must also complete this form for your new district. If you are enrolling in the Des Moines home school assistance program, you will fill out a different registration form that can be picked up at our office.
