New admitted students will receive an Immunization Health History Form with their admissions letter. This form is also available at USFP’s Office of Admissions and Financial Aid, or the form may be downloaded from the Student Health Services Forms page and printed on a laser or inkjet printer. Former USF students returning must submit an updated form, although your immunization records may already be on file. Students may verify this information directly with Student Health Services. Students who are completing their coursework or degree 100 percent online will not be required to submit proof of immunization but must still submit the form acknowledging the Meningococcal Meningitis and Hepatitis B vaccination information.
