Organizations in possession of the following certification/authorization: a) National Board “R” certificate (R-1 repair report form), b) ASME “S,” “H,” “PP,” or “U” certificate (IR-1 repair report form), c) Indiana “C-3” authorization (IR-1 repair report form). Note: In all cases of welded repairs, the certificate used must cover the scope of work performed. In all cases of welded repairs, an authorized (state-licensed) inspector must accept the contractor’s work by signing the appropriate weld repair form.
