The patient keeps the completed MOLST form. The form should be put where it is easy to see (on the refrigerator, beside the bed, or on the door). Copies of the MOLST form are also valid. Patients can keep a copy of their MOLST form in their wallet for trips outside the home, and give a copy to their health care agent and/or to a family member.
