Who is responsible for initiating the Individual Education Plan (IEP)?
The IEP should be reviewed formally once a year and at every transition point, including change of school, in the child or young person’s education. This is the same requirement as the NEP and can be done in conjunction with this review. However, short-term targets may require more frequent monitoring. This monitoring process may only require the attendance of the relevant personnel. In addition, close attention should be paid to student progress each term and relevant information and reports provided to carers, Families SA personnel and other stakeholders on a regular basis. Any change in circumstances in the care placement should lead to a review because carer information will need to be updated. Student goals may not need to change but the new carers will need to know what is in place and what is expected of them to support schooling.
