Who is eligible to complete the Biometric Health Screening and Personal Health Assessment?
Biometric Screenings are available at no cost to regular full-time and part-time faculty and staff (including SEIU employees). Non-Medicare eligible retirees and spouses/domestic partners who are enrolled in a University Health Care Plan are also eligible. The Personal Health Assessment (PHA) is available for regular full-time and part-time faculty and staff (including SEIU employees). Non-Medicare eligible retirees and spouses/domestic partners who are enrolled in a University Health Care Plan are also eligible. Faculty and staff members, Non-Medicare eligible retirees, and their spouses/domestic partners enrolled in a University Health Care Plan who complete the confidential on-line Personal Health Assessment will receive $100 cash incentive (subject to taxes) that will be credited to their paycheck.
