Who is eligible for the Healthy Living Rebates?
Regular active U.S. employees who elected to participate during the annual benefits enrollment period in the fall of 2010 are eligible. Rebate enrollment elections do not carry over from the previous year. These rebates are not available to supplemental employees, new hires in 2011, retirees, spouses/domestic partners, or other dependents. What if I elected to participate in the Physical Activity-Nutrition Rebate and/or Personal Vitality Rebate programs during annual enrollment, but that is not indicated on the Wellness for Life website? First, confirm that you are eligible. Eligible employees are regular active U.S. employees who elected to participate during annual benefits enrollment in the fall of 2010. Second, verify your employee serial number and last name are entered correctly on the Wellness for Life website.
