An individual is eligible for PSH if he or she: (1) has been diagnosed with an Axis I Serious Mental Illness; (2) has income that is at or below 30% Area Medium Income; (3) is referred by a DMH contracted Community Mental Health Center and (4) meets one of the seven defined priority population.
