OHSU Policy 10-01-015 requires the following groups of individuals to complete the CoI module annually in order to disclose any related outside activities requiring approval. These individuals are defined collectively as “disclosing employees.” Disclosing employees include: • OHSU Faculty members • OHSU Officers and Unit Leaders • Others serving in a management capacity at OHSU (including directors, supervisors, managers, and department administrators). • Persons who have delegated contracting authority or delegated fiscal authority over $100 • Other persons designated by an OHSU Officer or Unit Leader • If a non-disclosing employee has an outside activity requiring approval per OHSU policy 10-01-015, he or she may complete this on-line form and submit it his/her supervisor for approval. This on-line module also provides Executive CoI disclosure forms for OHSU Executives required to complete this additional disclosure form, per OHSU policy 10-01-021 .
