Who has had international usability issues because of their icons?

We recently published a white paper on a global iconagraphy review we did in over 109 locales for McDonald’s. The study specifically reviewed nutritional iconagraphy but I was wondering if anyone had experienced any unintended results due to icons they had employed in their software or packaging.
