Who fills out the forms?
Forms…What forms? The only FORMS you ARE required to fill out are ones Medicare mandates such as Assignment of Benefits and Privacy Practices. If you want to see a sample of those forms, click here. Many companies tell you there are No claim forms to fill out unless they are very small. In fact, Medicare doesn’t permit companies to send in Paper claim forms anymore. As part of the paperwork reduction act, all claims are filed by computer. So if we told you there are No claim forms, we would be simply stating the obvious fact that No provider or patient send paper claim forms in.
