I think Makoto truly loved Kotonoha in both the anime and the manga. It was because of his selfish desires that caused him to cheat on her and turn her into a mindless zombie. I think that he only used Sekai for sex and he didn’t really love her. Since what Makoto wanted Kotonoha wouldn’t give, so he went out to get it himself. Every girl was his sex doll except Kotonoha. When Kotonoha said that Sekai only wanted to think that Makoto was in love with her, Makoto didn’t say anything to defend Sekai. In the manga, Makoto wanted to see Kotonoha smile again. He also chose Kotonoha in the end and felt no shame in what he did to Sekai. So, I truely think that Makoto loves Kotonoha. He fell in love with her before he wanted sex.
