Who decides when a resident must move from one level of care to the next?
The goal at Bridges at Brookline is to keep residents at their most independent level of functioning as possible. Should changes in a resident’s health warrant a change in level of care, the decision to move to the most appropriate level of care is made with involvement from you, your family, your physician and the staff at Bridges at Brookline.
