Who decides the response for CANCEL request and how is it different from other requests?
Generally the final response for any request is generated by the TU layer. But for CANCEL the response is generated directly by the transaction layer iteself. The response is 200 OK in case the dialog represented by the CANCEL request matches any of the existing INVITE server transaction, if not, a negative resp (481) is generated.
