Who can refer a patient to hospice?

0

Anyone can make a referral to Mid-Fairfield Hospice – physicians and their assistants, the patient or a family member, friends (with the patient’s permission), clergy and other home health care agencies that do not offer hospice services.

0

Physicians, hospital discharge planners, clergy, friends and family members can all suggest or recommend hospice care. Patients may even request it for themselves. Contact the physician for a referral, or contact us directly and ask for the admissions department.
