Canadians who were at least 16 years of age when they applied for their last passport and who hold a five-year passport that was issued in Canada, as indicated under “Issuing Authority” on page 2 of the submitted passport. The passport must have been issued within the last six years, or if the applicant applied for their last passport by mail from the USA, the passport must have been issued on or after August 1, 2003. • The name on the application form must be exactly the same as the name on the submitted passport. • The passport must not be damaged and must never have been reported as being lost or stolen. Also, for Canadians living in the USA, the simplified passport renewal process can only be used to submit an application by mail or in person at a Passport Canada service location (in Canada).
