Who can fill in the questionnaire that is part of the DOSH Application?
The person who prepares your DOSH application should fill in the questionnaire. We always fill in the questionnaire for our clients. If you do not fill in the questionnaire yourself the person who does must be a CAC. Do I need a special certificate on my California Contractor’s license?
