Who are the community-based providers affiliated with Kaiser Permanente?
Kaiser Permanente contracts with community-based physicians and other medical professionals in Cuyahoga County (MetroHealth network), Lake County (Lake Health), Lorain County (EMH Regional Network), Medina County (Medina County network), and Stark and Summit counties (the Southern network). These providers, together with the Ohio Permanente Medical Group (OPMG) physicians who are based at Kaiser Permanente medical centers, are referred to as the Kaiser Permanente provider network, and constitute Tier One of the Added Choice (POS) product. All of these physicians are listed in our printed provider directory, which is included with enrollment materials and is also available online at kp.org/formsandpubs or by calling our Customer Relations Department toll free at 1-800-686-7100 or 1-877-676-6677 (toll-free TTY for the hearing/speech impaired).
