As long as your EEA fiance is not a UK citizen, in which case you would be subject to UK immigration law not EU immigration law, you need to apply for an EEA family permit. Yes you do need to apply from outside the UK. If you are already legally resident in the UK and in an immigration category that allows switching, you should apply for a EEA residence card. You can apply online from most countries. If your country supports applications online for UK visas, then yes you can apply for an EEA family permit online although you will also have to attend an interview.
