Which billing form is acceptable to IPNs payors?
The standard billing form for professional claims is the CMS 1500 and the UB-04 for facilities. Please follow the standard billing instructions and utilize the most current versions of CPT procedure and ICD diagnosis codes. Handwritten or faxed claims cannot be accepted by IPN as they do not provide the clarity required by IPN’s scanning equipment.
