Which agencies require that a PHS/NSF/AHA Conflict of Interest Form disclosure form be completed?
Investigators must submit Conflict of Interest forms for National Science Foundation, American Heart Association, and Public Health Service sponsors such as the National Institutes of Health (NIH), Agency for Health Care Quality and Research (AHRQ), Food and Drug Administration (FDA), Centers for Disease Control and Prevention (CDC), Health Resources and Services Administration (HRSA), Substance Abuse and Mental Health Services Administration (SAMHSA), and Department of Health and Human Services (DHHS). Investigators do not need to submit Conflict of Interest forms for applications being submitted to corporate sponsors, other federal sponsors; such as the US Army, Navy, Air Force, DARPA, NASA, Department of Energy and Department of Education; Commonwealth of Massachusetts agencies and private foundations other than American Heart Association.
