This can be for two reasons. The first is that no cover art was returned from the metadata provider. To ensure maximum cover art returns make sure that AMG is your first preference as metadata provider and that Download Missing Covers from Amazon is also selected. Secondly if cover art is embedded in the file but not visible in the album folder then it can mean that WMP is monitoring the output folder and therefore Windows regards the folder.jpg as a system file and hides it from view.
