A. The practitioner records his/her clinical decision in the clinical record. The record should include the reasons for the decision, identify the person’s mental health needs and any risk factors that need to be addressed, specify what will be done to address each identified need and risk factor, and document the outcome of the consultation with the authorised psychiatrist and the discussion with the patient and/or their carers about their legal status, their obligations and the action to take in the event of a relapse or crisis.
