Targeted Case Management for Med-Cal billing requires an IEP with an appropriate case management plan for the student that is reviewed at least every 6 months. Any nursing care plan should fulfill this requirement if documentation is done at least every 6 months and Medi-Cal TCM requirements are met. The ISHP can be attached to the IEP and it becomes part of this IEP as the case management plan for nursing.
