Where do (UK) Sat Navs get their maps from?

Many years ago, Ordnance survey created “Road Centre Lines” when the large scale mapping was first digitised. It was not always obvious whether a thoroughfare was a road or a track. It didn’t matter at the time because Sat Nav hadn’t been created. Sat Nav manufacturers use Road Centrelines. A lot of effort is spent improving the data but it takes time. I suggest you contact OS and make a comment. They may well have changed the data but it doesn’t mean that the manufacturers have.
