All of Lake Forest is in Pittsfield Township Precinct 9. Pittsfield Community/Senior Center Building 701 W. Ellsworth Rd (SW corner of State and Ellsworth) PLEASE NOTE: A PHOTO ID WILL BE REQUESTED PRIOR TO OBTAINING A BALLOT. Showing your voter registration card is no longer sufficient. If you do not have a photo ID, there is an affidavit you must fill out and sign prior to receiveing your ballot.
