Read the pre assessment notes first then you will have a foundation to work on. I assume your client will be allocated a keyworker so they will be of benefit to you here. You can complile a care plan from when the client is admitted but I found it could take around a week or longer to complete in full as you need to find out what their personal choices and medical needs are (some pre assessment notes are not always the real mc-coy unless you’ve done this yourself) Involve the client as much as possible, liase with staff who are attending to their daily needs and some relatives can be a great source of information/assistance especially with the Advanced care plan. This gives a true plan for staff to follow then review and update regularly with any changes. Always remember, the client should be making the decisions wherever possible and the plan should be focused on their choices and very person centred.
