Where do I send the Termination Notice once a minor has separated from employment?
If a minor in your business has quit, stopped showing up or been fired, the issuing officer at the school that issued the work perit must be notified immediately and in writing. This is typically done using the Termination Notice attached to the Work Permit. Please mail, fax or hand deliver the Termination Notice back to the school that issued the work permit.
