If you live close to home, you should contact your Primary Care Provider to have the physical examination sheet (the yellow form in your packet) filled out and signed, and to inquire about how to get your TB test completed. If you do not have a Primary Care Provider in the area, you may contact the KCON Nurse Managed Family Health Center, which is located on 72 Sheldon in Grand Rapids.
