The NSW Proof of Age card has been replaced by the NSW Photo card and you still get them from the RTA: http://www.rta.nsw.gov.au/licensing/phot… The application form is RTA 1359: http://www.rta.nsw.gov.au/publicationsst… Simply return the completed form to the motor registry with appropriate proof of identity documents, proof of residential address and the administration fee which is $44.
