The Internal Revenue Service issues federal employer identification numbers (EIN). Application may be made on-line at http://www.irs.gov/businesses/small/article/0,,id=102767,00.html OR you can request a form SS-4 (EIN application) and instructions by calling the IRS at 1-800-TAX-FORM (1-800-829-3676).
