Where do I find the NDC and lot number to apply for a refund or replacement coupon?
You can find both the NDC number and the lot number on the bottle label. The NDC number is found directly above the Hyland’s logo. The lot number is imprinted on the far right hand side of the label. You will also find these numbers on the carton, if you have kept the outer packaging. The NDC number is found above the Hyland’s logo on the principal display panel: it starts with five numbers, followed by a dash and four numbers, followed by a dash and a single digit. The lot number is also imprinted (but perhaps harder to read) on the flap of the right hand side panel. The UPC code is found on the underside of the carton. The UPC code is not needed to apply for a refund or coupon.
