The form is also available in the “Florida Unemployment Compensation Program” booklet which was mailed to you when you filed your claim by phone. You must complete the form and mail or fax it to the address or fax number listed on the form. It will take about 10 days for the changes to take effect. The request for change cannot be made by an email or telephone request. The same form can be used if you decide that you no longer want to have withholding taxes taken out of your benefit check. You may only change your request a maximum of two times per calendar year.
