Where can I obtain an application for a vote by mail ballot?
You must make a request in writing. We prefer that you use the postcard application in the sample ballot that is sent to you because it has bar coded information on it that helps us to process requests faster. It is not necessary to wait for your sample ballot to request a Vote By Mail ballot. Your may send a card or letter to the Election Dept. requesting a Vote By Mail ballot. Send applications to: Election Department PO Box 271 Martinez, CA 94553-0027 You may also fax your request to: (925) 335-7838. You cannot make a request by telephone.
