You must make a request in writing. We prefer that you use the postcard application in the sample ballot that is sent to you because it has information on it that helps us to process requests faster. It is not necessary to wait for your sample ballot to request a Vote-by-Mail ballot. You may mail or fax a request in writing to the Elections Department requesting a Vote-by-Mail ballot. See next question below for information needed.
