GAP entry blanks may be found in the Tournament section of the Tournament Schedule and Members Information Booklet, or behind the “Tournaments” tab on this site. USGA entry blanks may be obtained directly from www.usga.org or from the GAP Office. Pennsylvania Golf Association entry blanks may be obtained at member clubs, at www.pagolf.org or from the GAP Office.
