Where can I obtain a “Paratransit Eligibility Certification Form”?
Forms, created in large print, are available at: MetroLINK Administrative Offices 1515 River Drive, Moline, IL 61265 The form is available on audiotape and in Spanish and Braille at the MetroLINK office.
