Claim forms are available on the school district web site and in the Office for Human Resources at the Educational Services Center, 18551 East 160th Avenue, Brighton. On the web site, click on “Employees” and then click on “District Forms – HR Department.” Click on the appropriate form and print. Follow the directions on the back side of the claim form and mail to the address at the bottom of the claim form along with your receipts. Reimbursement checks are issued twice a month on the 15th and the last business day of the month. For questions about claims, please call American Administrative Group at 1-800-821-8197.
