Where can I get my bank Transit number, and institution number (TD CANADA TRUST)?

The transit number is a number identifying your branch. The number on your Access Card is not relevant. If you do on-line banking, log into Easyweb, and look at the View Accounts page. The numbers identifying your bank account will have a short part (3-4 digits), then a blank, then a longer part. The short part is your transit number, the way TD Canada Trust uses it, and then the longer part is your account number. The transit number needs to be five digits. If yours is three, try adding a zero on either end. As for the institution number, I think TD Canada Trust is 004. If these don’t work, telephoning the bank would certainly be the easiest way to get this information. Your reluctance to telephone makes me wonder whether this is actually your account. Why don’t you go into a branch of the TD Canada Trust, taking your Access Card with you, and ask them for help at the customer service desk?
