Where can I get more SHPO rehabilitation (survey) cards? How long will it take to receive them?
Email our office at ER@michigan.gov to request more cards, or call us at 517.373.1360. Include a contact name, the name of your organization, a mailing address, and the approximate number of cards you need. We try to send the cards out as soon as possible, however it may take up to two weeks to receive the cards because of internal issues within the state mail system.
