Where can I get a Category 1 activity notification form?
The only Category 1 activity that can be lodged with QA&CPD using a notification form is an ALM. All ALM notifications need to be submitted through the Provider online services in the 2008–2010 triennium (therefore MS Word or Adobe PDF version of an ALM notification form are not available on the website). Ongoing Provider organisations and EARs received their personal login details in an email sent late October 2007. New EARs and Provider organisations receive their login details at the completion of the training or contract process. Contact your state QA&CPD Unit if you do not know your login details. ‘Lodge ALM notification’ appears in the main side menu after you login and takes you to the online form. To submit an ALM notification online, the associated organisation needs to have a 2008–2010 AP contract and the EAR linked to the organisation needs to have successfully completed AP training for 2008–2010.
