Visit the CMS website at http://www.hcfa.gov/medicare/enrollment where you can obtain information on the enrollment forms and our manual instructions, as well as the name, address, and telephone number of the Medicare contractor with whom you plan to do business. Otherwise, I will gladly transfer you (or give you the telephone number of) our provider enrollment unit.
