Where can I find the Medicare Physician Fee Schedule Data Base (MPFSDB) Indicator Key?
Go to the CMS Internet Only Manuals (IOMs) and locate the Claims Processing Manual (Pub. 100-04), select Chapter 23 and scroll down to section 90 (page 121).
