Where can I find the list of dentists approved for the district’s self-funded dental plan?
Employees covered under the District’s self-funded dental plan can go to any licensed dentist anywhere in the United States. The employee must take a dental claim form to the dentist. The District 27J Dental Plan Claim Form can be located on the District 27J web site at www.sd27j.org. Click on the “Employees-Login” tab at the top of the home page, then click on “HR Forms – HR Department” on the left side of the page. A list of available forms will be shown. Select both the Dental Claim Form front side and back side. Dental Claim Forms are also available in the Office for Human Resources at the ESC.
