Where can I find the forms I need to fill out for Operator Designation, Facility Configuration, Individual Training Reports, Change of Address, etc.?

On the Operator Certification website under the “Forms” tab all the forms needed for the operators and facilities are can be found. The only exception is the upgrade forms for water and wastewater to Level 1 licenses. Please contact our office for those forms at 307-777-6128.
